  2. Many Filipino travelers who cannot show their personal capacity to shoulder the costs of the trip may present a notarized affidavit of support and/or guarantee executed by their sponsoring relative or friend who are residing or working in the country they are traveling to. However, the affidavit of support does not guarantee clearance for departure because each traveler will still have to be assessed by the immigration officer based on the totality of his/hers circumstances, including personal capacity to travel, such as appearance, demeanor, current employment, financial capability, etc. at the time that he/she presents himself/herself for departure formalities.

Requirements for Notarization and Authentication of Documents

Per new DSWD guidelines (, minors traveling abroad whose parents are living abroad (residents of HK SAR) or are in the foreign service need not secure the Affidavit of Consent and Support (AOCS) and the DSWD Travel Clearance provided, that the minor holds a valid dependent's or permanent resident visa and has a valid passport. If not, the AOCS is being required by the DFA for the passport application processing. Also, minors traveling with either parent or solo parent or legal guardian are exempted from securing the Affidavit of Consent and Support (AOCS) from the Consulate and the Parental Travel Permit (PTP) from the DSWD.

6. Authentication 

      a.  Documents issued in the Philippines for use in HK

        • Original document authenticated by authorized authentication offices in the Philippines (issuing agency / DFA)
        • Photocopy of document

      b. Documents notarized in Hong Kong for use in the Philippines

        • Original notarized document and photocopy thereof
        • Proof of authority of the notary public

      c.  Documents issued by Hong Kong Government official(s)

        • Original document and photocopy thereof


    1. Certified Copy of an Entry in a Register of Deaths issued by the Births and Deaths Registry, Hong Kong SAR or Certificate of Fact of Death issued by the Coroner’s Court, Hong Kong;
    2. Duly notarized certificate issued by the Mortuary or Funeral Director (2nd paragraph should be included if no separate Certificate of Non-contagious Disease is issued)
    3. Transit Permit issued by the Immigration Department, Births and Deaths General Register Office, Hong Kong; and
    4. Fee: HK$ 200.00



All the above documents (one set original, 3 sets copies) must be submitted to the Consulate prior to the issuance of Consular Mortuary Certificate and shipment of the remains to the Philippines. (For remains originating in Shenzhen or Guangzhou to be brought to the Philippines via Hong Kong, a Death Certificate duly authenticated by the Consulate General in Guangzhou must be presented together with the other requirements - items 2-4 above)

The original passport of the deceased must be submitted for cancellation.

Sealing of casket or urn by a Consular Officer is no longer required.

8.  Requirements for Bringing the Urn Containing the Cremated Ashes of the Deceased to be Buried/Deposited in the Philippines:

      • Documents for Authentication with the Philippine 
          1. Certified of Cremation
          2. Authorization to remove urn containing the ashes of the deceased
          3. Fee: HK$200.00

15.  Philippine Procedure in the Importation of Live Pet Animals from Hong Kong

  1. Documentary Requirements:

    1. Secure a Sanitary Phytosanitary (SPS) Import Clearance for live pet animals (dogs & cats only)

      1. Download the SPS Import Clearance application form from the Bureau of Animal Industry (BAI) (Application for the Importation of Pets)

      2. Email the accomplished form to the Bureau of Animal Industry at This email address is being protected from spambots. You need JavaScript enabled to view it.

      3. Once approved, the BAI will send an email notification of your SPS Import Clearance, which will have a validity of 2 months.

    2. Secure an International Veterinary Health Certificate (IVHC) from the proper veterinary authority of Hong Kong stating that the animals have been carefully examined and found to be free from and have not been exposed to dangerous communicable animal diseases.

    The Veterinary Health Certificate must be duly authenticated by the Philippine Consulate General through its Notarial Section. Contact information are as follows:

    Requirements for authentication:

    1. Original and photocopy of the International Veterinary Health Certificate duly stamped by the Hong Kong Agriculture, Fisheries & Conservation Department.

    2. Authentication fee of HK$200.00. Processing time is two (2) working days. Should you wish to expedite the release of the authentication of the Veterinary Health Certificate, an additional HK$80.00 will be charged and the document will be released the following working day.

  2. Present the Veterinary Health Certificate and the SPS Clearance issued by the BAI upon arrival at the Veterinary Quarantine Unit in any of the Philippines’ airports. Fee for SPS Import Permit is Php100.00 while the landing inspection fee for the first two heads is Php250.00 and Php300.00 for each succeeding head. Fees are subject to change without prior notice

Processing period for all notarial services is three (3) working days following the date of the application . For example, an application submitted on Sunday, June 20, 2010 will be released on Wednesday, 23 June 2010. An application submitted on Wednesday, 23 June 2010 will be released on Monday, 28 June 2010.

The Philippine Consulate General is open to the public from Sundays to Thursdays and closed on Fridays, Saturdays and all statutory holidays of the Philippines and Hong Kong (please click here for the list of statutory Philippine and Hong Kong holidays observed for CY 2010).
