ACKNOWLEDGEMENT, JURAT, AND OTHER NOTARIAL SERVICES
Below are the basic requirements in availing notarial services. The Consulate reserves the right to require the applicant to produce additional documents, as applicable to his/her case, to help establish his/her identity, and other data in his/her application.

Importation of Live Pet Animals from Hong Kong to the Philippines
(Notarization of Affidavit of Undertaking for Importation of Live Pet Animals to the Philippines)

- Original and 1 photocopy of the Affidavit of Undertaking (follow the format prescribed by the Philippine Bureau of Animal Industry)
- 2 photocopies of other documents required by the Bureau of Animal Industry as attachments to the Affidavit (Photo of the pet, vaccination record, microchip ID etc.)

Fill out the registration form at the InterCommerce Network Service (ICNS) website https://www.intercommerce.com.ph/registrationbai.asp After registrationyou can log-in to your account by using your registered email address and password for the SPSIC Application (import permit) at the same website. |

Fill out the Affidavit of Undertaking for One-Time importation or Commercial Importation. Submit the Affidavit at the Philippine Consulate General Hong Kong together with the other requirements specified on section B. (For notarization of the Affidavit, please refer to the requirement for Notarization of Affidavits on this page.) Once you have received the notarized affidavit, proceed to the application for the SPSIC by uploading all the requirements in the ICNS website. |

After 1 to 2 working days, check status of application by logging-in to your account. (The status could be (1) For endorsement which means the application is waiting for the signature of the approving authority, (2) Approved, or (3) Rejected which means the applicant will have to re-apply). Once approved, print the SPSIC and present this at the Quarantine Station in the port of entry / airport in the Philippines, together with the International Veterinary Certificate / Veterinary Health Certificate and airway bill for validation purposes. *Approval of the SPSIC usually takes 1 to 2 days. Approved SPSIC is valid for 60 days. |

B. REQUIREMENT IN THE APPLICATION FOR SPI

• Internatopnal Veterinary Certificate / Veterinary Health Certificate from the competent authority in Hong Kong and other commodity specific requirements, including permits/clearance from other concerned agencies
B.1 For Filipino / Local Omporters, the NAOU should include the following details:
B.1.1. IDENTIFICATION OF IMPORTER
Name of importer, address, BIR-issued ID with TAX identification Number (TIN) of the client and quarantine site of pets
B.1.2 ANIMAL HEALTH INFORMATION AND IDENTIFICATION
Personal details of the subject dog/s and/or cat/s (Information should include - species, breed, sex, color, age, name of owner / exporter from the country of origin, address or exporter
Proof of Microchip Number of the subject dog/s and/or cat/s (must be embedded in the skin of the dog/s and /or cat/s
Vaccination and Deworming Records (original or Certified True Copy) of the subject dog/s and/or cat/s (front page and inside page with updated vaccination and deworminPet Passport (personal details of the subject dog/s and/or cat/s), if applicable
Photograph/s of the subject dog/s and/or cat/s
Pet Passport (personal details of the subject dog/s and/or cat/s), if applicable
B.1.3 For Foreign Importers, documentary proof of the abovementioned requirements as written above shall be included as attachments to the NAOU or Undertaking for Foreign Importers. The abovementioned documents shall be uploaded in the ICNS website in the application for the SPSIC.
For Pet owners (One-Time Importers and / or Foreign Importers), documentary proof of the abovementioned requirements as written above shall be included as attachments to the NAOU or Undertaking for Foreign Importers. Upon securing the requirements A and B in the procedure, the applicant shall proceed to the Hong Kong High Court Registry for Apostille service.
Present the Veterinary Health Certificate and the SPS Clearance issued by the BAI upon arrival at the Veterinary Quarantine Unit in any of the Philippines’ airports. (Please check corresponding fees for SPS Import Permit with BAI as the fees are varied depending on the number of pet/s to be imported to the Philippines).
