Filipino Tourists who lost their passports in Hong Kong SAR shall be issued a travel document in lieu of a passport. They should submit the following documents for the issuance thereof:
Duly accomplished application form available here or at the Philippine Consulate 
Photocopy of Police Report 
Affidavit of Loss (in duplicate; form obtainable at the Information Counter of the Consulate)
Photocopy of airline certification on arrival in HK SAR 
Copy of any identification card issued in the Philippines or previous Philippine passport Copy of ticket/ certification of confirmed return flight to the Philippines
Four (4) photographs (please refer to specifications below)
   
Filipino Contract Workers or HK residents, on the other hand, are required to present the following:
Duly accomplished application form available here or at the Philippine Consulate
Photocopy of Police Report 
Affidavit of Loss (in duplicate; form obtainable at the Information Counter of the Consulate)
Copy of lost passport or a certified true copy of Birth Certificate issued or authenticated by the National Statistics Office (NSO)
Copy of HKID Card
   
Note: 
Replacement passport may be issued only after the lapse of FIFTEEN (15) working days from the date of submission of the Affidavit of Loss.
