Lost Passport
| Filipino Tourists who lost their passports in Hong Kong SAR shall be issued a travel document in lieu of a passport. They should submit the following documents for the issuance thereof: | |

Duly accomplished application form available here or at the Philippine Consulate |

Photocopy of Police Report |
|
Affidavit of Loss (in duplicate; form obtainable at the Information Counter of the Consulate) |
|
Photocopy of airline certification on arrival in HK SAR |
|
Copy of any identification card issued in the Philippines or previous Philippine passport Copy of ticket/ certification of confirmed return flight to the Philippines |
|
Four (4) photographs (please refer to specifications below) |
| Filipino Contract Workers or HK residents, on the other hand, are required to present the following: | |

Replacement passport may be issued only after the lapse of FIFTEEN (15) working days from the date of submission of the Affidavit of Loss. |
